How to prepare PDF's for mapping text inputs, yes/no toggle, select options, radio buttons, and checkboxes

Note: You need Adobe Acrobat Pro to be able to prepare PDF's

To prepare PDF's for mapping, you first have to convert your file into PDF format. To convert a Microsoft Word document into PDF, you should utilize the "Create and Share Adobe PDF" feature in Microsoft Word which will allow you to convert and utilize Adobe to auto-create text boxes and inputs.

Once you have a PDF ready, go to Tools and then Prepare Form

Click Start

Once the PDF ready, you will get a document that will have various options. If you don't get these options next to your inputs you can also add them by clicking the menu options below:

Text Input

With a text input, all you need to do is label the input with a unique label and drag and drop the input to the desired height and width and you can also specify formatting. Note: if you label a text input with a similar name to another text input, both text inputs will be mapped together

Map the text input to the form by selecting the page and input that corresponds with the text input

Yes/No Toggle

For Yes/No Toggles, you can use these with checkboxes or radio buttons. First of all, similar to select options and radio buttons when you have a group of choices and each choice can only be selected once (are mutually exclusive) you need to make sure all the options in this group have the same label. Second, you need to make sure that you distinguish the options for Yes with "yes" and No with "no" as follows:

Click on each checkbox to get the properties below:

Yes option = "yes" all in lowercase in Options tab

Make sure the label is the same for the No option as the Yes Option to group them together

Select Options

For Select Options, you can use these with checkboxes or radio buttons.

First of all, similar to Yes/No and radio buttons when you have a group of choices and each choice can only be selected once (are mutually exclusive) you need to make sure all the options in this group have the same label.

Second, you need to make sure that you match the options in the form builder exactly so Adobe knows which option to select.

Radio Buttons

For Radio Buttons, you can also use these with checkboxes or radio buttons.

First of all, similar to Yes/No and Select Options when you have a group of choices and each choice can only be selected once (are mutually exclusive) you need to make sure all the options in this group have the same label.

Second, you need to make sure that you match the options in the form builder exactly so Adobe knows which option to select.

Checkboxes

For checkboxes, you can also use these with checkboxes or radio buttons.

First of all, you will want all the options to have a unique name since this is the only input type that you can select more than one option.

Second you need to make sure that all the options are set to "true" which is only selected when you click on a box in the form. A common mistake is to try to set the value similar to the input label instead of "true" for a checkbox option.

Last but not least, make sure to add a PDF input to the form and select the PDF you want to display
